1. The registration fees to the Department Convention shall be Seven ($7.00) dollars. Advance registration fees shall be Five ($5.00) dollars provided that such fee, accompanied by properly executed Official Credentials form, is received by the Department no later than fifteen (15) days prior to the opening date of the Department Convention.

INSTRUCTIONS: Complete the information below. You can enter up to fifteen (15) delegates in one session. Submit your delegates and payment. An email confirmation will be sent acknowledging the transaction.
